Adult Day Care Centers provide therapeutic programs of social and health services as well as activities for adults in a non-institutional setting. Participants may utilize a variety of services offered during any part of a day, but less than a 24-hour period.
- Complete ADCC Specialty form
- Complete Food Safety Permit
- Complete Fire & Health Inspection
- Submit Application to State
- Complete Survey
- If offering specialized Alzheimer’s and Dementia Care must complete Additional Requirements Forms.
- Obtain Company National Provider Number
- Obtain Company Liability Insurance
- Obtain Business Tax Receipt
- Complete Policy & Procedure
- Complete Business Operation Documents
- Complete Financial Affidavit
- 2hr Coaching
